Defines medical record and provides way for patient or patient's representative to submit request to examine or obtain copy of medical record.

Declaration for Mental Health Treatment. Includes a provision allowing agent to access principal's health records in case of emergency.

CYBERSECURITY REQUIREMENTS FOR INSURANCE COMPANIES

Commitment of mentally ill person is confidential unless person identified, or the person's legal guardian, if any, or if the person is a minor, the person's parent or legal guardian, consents, and if the disclosure is in the best interests of the person, as may be determined by the court for judicial records and by the chief clinical officer for medical records.

Each hair care facility shall maintain medical records as necessary to verify the information and reports required by statute or regulation for at least six years from the date of discharge.
