Ohio Name Change & ID Law: Birth Certificate

On 11/19/24, the Ohio Supreme Court left the appellate court decision intact because the Ohio Supremes could not reach a decision. Essentially, local probate courts can choose to accept gender markers changes on birth certificates or not.

Clark County Probate Court refused to change gender marker on birth certificate.

Filing and registration of birth certificate

Delayed birth certificate

Issuance of new or foreign birth record after adoption - access to original record, adoption file.

Filing court order of change of name - new birth certificate.

Registration of unrecorded birth or correction of birth record.

Copies of vital records.

The office only issues certificates for births that occurred in Ohio from 1908 – present.
